7/14/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by her facial such as a smile on the face. QMHP-C inquired about her day. Ashiyah discussed with QMHP-C that her day was going well, she and her mother still be arguing about stuff but, âThat is not going to changeâ. QMHP-C informed Ashiyah that this session was going to consist of a family session. 2. Behavior: During the second portion, Ashiyah appeared to be a little withdrawn from the session at times as she was getting frustrated with QMHP-C and her mother when discussing her behaviors. 3.Behavior: Ashiyah demonstrates ability to hold herself accountable and adapt to new challenges. Ashiyah was reminded to utilize relaxation techniques and breathe so that she could stay calm and focus in the session. Ashiyah expressed needing more help and agreed to be less emotional and more logical with handling stressful situations at home during this time. Ashiyah was appreciative of the therapeutic services and having someone to depend on that understood her on serious and personal level. 1.Intervention: QMHP-C began the session by inquiring about Ashiyahâs current thoughts, feelings and emotions. QMHP-C informed both Ashiyah and Ms. Hodge that this session was going to focus on building a positive parent-child relationship, parenting skills and identifying needs. QMHP-C engaged Ashiyah and her mother in a discussion to allow each other to express their feelings at the present time. QMHP-C used modeling and role playing to teach the mother effective ways to talk to Ashiyah to allow Ashiyah to communicate her needs appropriately. 2. Intervention: QMHP-C instructed Ashiyah to identify any challenging situations that she encountered with her family members or friends prior to this session. QMHP-C rewarded Ashiyah with an outing encouraged her to utilize learned coping skills from sessions to assist her with current and future situations 3. Intervention: QMHP-C identified activities for Ashiyah and her mother to promoted connectedness. QMHP-C engaged Ashiyah in an activity to promote connectedness. QMHP-C monitored the dynamics between the two as they participated in the activities. QMHP-C emphasized the importance of practicing parenting skills consistently. 1. Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Ashiyah and her mother expressed feelings of agitation and frustration. Ashiyah continues to harbor embarrassing feelings regarding her motherâs behavior and her home life. She demonstrated inappropriate gestures towards her mother in the manner of rolling her eyes and sucking her teeth 2. Response: Ms. Hodge has little knowledge of the needs that Ashiyah wants met. Ms. Hodge is inconsistent with setting boundaries and limits with Ashiyah in the home. Ashiyah uses her motherâs lack of consistent parenting skills to manipulate situations in the home. 3. Response: Ashiyah was able to share insight on her knowledge of conflict-resolution as she provided QMHP-C with ways in which skills were utilized. Ashiyah was receptive to corrective feedback was provided by QMHP-C throughout the session. Ashiyah was commended by QMHP-C for identifying challenging situations and strategies to overcome them.
